Lip Filler Migration - Why It Happens & How To Fix It

As we move toward a more refined, natural-looking era of injectables, we’re seeing a significant shift in how we manage the lips.


The goal isn't just volume - it’s structure and boundary. However, one of the most common issues practitioners face today is lip filler migration.


Understanding why this happens, and how to fix it, is a vital skill for any injector who prioritises patient safety and client satisfaction.

The Anatomy Of Migration


When we talk about lip filler migration, we're describing the movement of hyaluronic acid filler from the vermilion body or border into the perioral area (the cutaneous lip). You'll usually see this as a loss of the crisp, sharp definition of the Cupids bow, or a physical 'shelf' that catches the light above the upper lip.


From a clinical perspective, the lip is a high-pressure, high-movement area. Every time a client speaks, eats or smiles, the orbicularis oris muscle is put to work. If filler is placed incorrectly or in excessive volumes, this constant muscular contraction acts as a pump, slowly pushing the gel into the surrounding tissue.



The Leading Causes Of Lip Filler Migration


For a medical professional, identifying the root cause of migration is the first step in educating the client on the necessary 'reset'.


Volume Overload (The 'Envelope' Theory)


The most common cause of lip filler migration is when filler exceeds the tissue’s capacity. The lip has a specific anatomical 'envelope'. When we inject too much product in a single session - or top up too frequently - the interstitial pressure becomes too high. The filler will always move toward the path of least resistance, which is often upwards and outwards into the skin above the lip.


Poor Placement


Placement is everything when it comes to long-term results. If the filler is placed too superficially, it sits just above the muscle where it’s easily displaced by everyday facial expressions. On the other hand, injecting too deeply or failing to respect the natural boundaries of the vermilion border is a common pitfall. This allows the product to seep into the submucosal space or the cutaneous portion of the lip, which is exactly how you end up with visible lip filler migration.


The Cumulative Effect


Hyaluronic fillers,, while temporary, can often last much longer than the advertised 6–12 months. When new filler is stacked on top of old, partially degraded product, the cumulative volume eventually leads to lip filler migration. This is why a thorough history of previous treatments is the most important part of your consultation.



How To 'Fix' Lip Filler Migration


When a client presents with lip filler migration, the solution is rarely to add more product to balance it out. Adding more filler to a migrated lip is like building a house on a bad foundation - it only amplifies the problem.


The 'Dissolve First' Approach


At Skin Masters, we advocate for the 'Dissolve and Refill' protocol. We use Hyaluronidase to break down the misplaced hyaluronic molecules and return the anatomy to its natural state. This is a medical procedure that requires a skin patch test and a deep understanding of enzyme activity.


We advise waiting a minimum of two weeks after dissolving before re-injecting. This allows the tissue to settle, the edema to resolve and ensures the Hyaluronidase is no longer active, so it won’t degrade your fresh filler.


Refined Techniques For Re-filling


When you do come to refill a previously migrated lip, your technique must be meticulous to prevent lip filler migration from recurring.


  • Use Micro-droplets - Small, controlled boluses or linear threads are less likely to create high-pressure zones.


  • Respect the Border - Avoid 'tunnelling' across the vermilion border unless absolutely necessary for definition.


  • Focus on Integration - Choose a product with high tissue integration that moves naturally with the patient’s smile.
